Coordinated Family Support

Helping adults stay in the home they’ve chosen, with the support that makes it work for the whole family.

Inspired Living’s Coordinated Family Support (CFS) program coordinates services and supports for adults with developmental disabilities who live in their family home. Every plan is built around the individual and tailored to their family’s needs, language, and culture. A dedicated CFS Case Manager works with each person and their family to make sure the right supports are in place. That means scheduling medical and therapeutic appointments, sorting out transportation, and connecting families with community and generic resources. We also help with futures planning, including for families with aging caregivers, and provide backup support coverage while longer-term solutions are put in place.

What’s included:

  • Identifying and coordinating the supports needed to live successfully in the family home

  • Navigating and accessing community resources and generic services (with backup direct support for up to 90 days while longer-term help is found)

  • Coordinating consistent training across everyone who supports the individual

  • Scheduling medical, dental, and therapeutic appointments

  • Transportation planning

  • Futures planning, including for aging caregivers

  • Training that builds independence

Who it’s for: Adults 18+ who are eligible for Regional Center services and live in their family home.

Service area: Alameda and Contra Costa Counties (Regional Center of the East Bay catchment area).

Languages: Services are available in English and Spanish, with a focus on delivering support in the language and cultural context each family is most comfortable with.
